Job Details

We have an exciting opportunity for a Field Case Manager to join us on a fixed term contract basis to cover maternity leave. The Field Case Manager is responsible for coordinating and managing comprehensive care plans for patients, ensuring optimal health outcomes and efficient healthcare service delivery in a field-based setting.

Responsibilities

  • Conduct patient assessments to identify needs, develop individualized care plans, and establish realistic goals.
  • Coordinate and facilitate access to healthcare services, including specialist referrals, therapies, and community resources.
  • Monitor patient progress, evaluate the effectiveness of care plans, and make necessary adjustments.
  • Educate patients and their families on disease management, medication adherence, and healthy lifestyle choices.
  • Act as a liaison between patients, healthcare providers, and insurance companies to ensure seamless care coordination.
  • Maintain accurate and timely documentation of all patient interactions, care plans, and outcomes.
  • Adhere to all regulatory guidelines, company policies, and ethical standards.
  • Participate in team meetings, case conferences, and professional development activities.
  • Travel within a designated geographical area to conduct home visits and attend appointments.

Education:

  • Bachelor's degree in Nursing (BSN) or a related healthcare field required; Master's degree preferred.
  • Current and unrestricted Registered Nurse (RN) license in the province of employment.

Experience:

  • Minimum of 3-5 years of clinical experience, preferably in case management, home health, or a related field.
  • Strong understanding of healthcare systems, insurance processes, and community resources.

Skills:

  • Excellent assessment, planning, and critical thinking skills.
  • Exceptional communication, interpersonal, and negotiation abilities.
  • Proficiency in using electronic health records (EHR) and other relevant software.
  • Ability to work independently and manage a diverse caseload effectively.
  • Valid driver's license and reliable transportation required.
  • Bilingualism (English and French) is considered an asset.
