Employees may also perform other duties as assigned.Employees must abide by all Joint Commission Requirements including but not limited to sensitivity to cultural diversity, patient care, patient rights and ethical treatment, safety and security of physical environments, emergency management, teamwork, respect for others, participation in ongoing education and training, communication and adherence to safety and quality programs, sustaining compliance with National Patient Safety Goals, and licensure and health screenings.Must perform all duties and responsibilities in accordance with the hospital's policies and procedures, including its Service Standards and its Code of Conduct.* Maintains appropriate contacts and working partnership with designated County Public Health, pediatric primary care providers and clinics, and public and private agencies to support medical homes, developmental screening, and appropriate referral and follow-up for children and youth. * Identifies and outreaches to pediatric primary care practices and clinics in designated County that serve a high volume of low-income children. * Trains providers and office staff on medical home concepts, developmental screening, and referral pathways for children and their families. * Identifies systems barriers to provision of medical homes and developmental screening and identifies potential solutions. * Conducts regular site visits to practice sites to provide technical assistance, additional training, and consultation for pediatric providers and staff on medical home concepts, developmental screening, and referral pathways. * Assists in design, revision and distribution of medical home materials and other educational materials for pediatric providers, including updates on program, systems, and policy changes that affect pediatric practice and services to children and families. * Participates in the collection, consolidation and assessment of program data for the purpose of monitoring the results and overall effectiveness.
