Advanced Pharmacist - Infectious Disease (Per Diem) - Atlantic Health Overlook Medical Center

Job Description:

• Reviewing patient medication regimens and providing recommendations to optimize therapy based on clinical guidelines and best practices. 
  Conducting medication reconciliation to ensure accuracy and safety during transitions of care.
• Educating patients and caregivers on proper medication use, side effects, and adherence strategies. Collaborating with healthcare providers to develop and implement patient-specific care plans.
• Monitoring patient outcomes and adjusting therapy as necessary to achieve therapeutic goals. Participating in interdisciplinary rounds and providing pharmacotherapy recommendations.
• Providing drug information to healthcare professionals and patients as needed.
• Contributing to quality improvement initiatives related to medication use and patient safety. Documenting interventions, clinical assessments, and recommendations in the electronic health record.

• Staying current with developments in pharmacotherapy and evidence-based medicine. Serve as a preceptor and mentor for pharmacists, pharmacy residents and students as needed.

   

Infectious Disease:
•Actively view trends in antimicrobial utilization and antimicrobial resistance patterns.
•Lead infectious disease section meetings with Infectious Disease section medical Chief .
•Participate in site and system wide Infection Prevention and Antimicrobial Stewardship meetings.
•Provide consultative service in infectious diseases pharmacotherapy and pharmacokinetics, examine the pharmacoeconomic impact of providing care to patients
•Participate in drug therapy evaluations both prospectively and retrospectively
•Demonstrate knowledge of various drug information resources and be able to research and interpret data and information
•Build annual site-specific antibiogram
•Participate in quality, safety and other project work related to infectious diseases
•Conduct medication use evaluations and research projects in collaboration with other healthcare professionals to identify areas of medication use optimization with publication and/or presentation at local and/or national meetings
•Precept pharmacy residents and pharmacy students

Education/Experience:

• Graduation from an accredited School of Pharmacy with PharmD required
• Current active registration and licensure with the State of New Jersey Board of Pharmacy required
  Previous hospital pharmacy experience (2+ years) preferred.

• Works majority of time in a given specialty PLUS has more than 1 year of experience in a specialized area
  OR
  Works more than 1 year in a specialized area PLUS has PGY-1 Pharmacy Residency
  OR
  Has PGY-2 Pharmacy Residency in a specialized area
