Job Summary

The MDS Coordinator ensures accurate and compliant completion of the Resident Assessment Instrument (RAI) process to support appropriate Medicare and Medicaid reimbursement. This role oversees MDS assessments, documentation accuracy, care plan coordination, and regulatory compliance. The MDS Coordinator plays a key role in maintaining high‑quality resident care, supporting interdisciplinary collaboration, and ensuring reimbursement aligns with clinical delivery and federal/state guidelines.

Qualifications

• Current, valid Texas nursing license (RN or LVN)

• At least 2 years of longterm care experience preferred

• Acknowledgement of Completion Certificate through HHSC RUG Online Training for Nursing Facilities

• Must complete AANAC RAI Certification within 1 year of employment

• Strong communication, documentation, and delegation skills

• Ability to read, write, analyze, and interpret clinical documentation, regulations, and technical procedures

• Ability to write reports, business correspondence, nursing progress notes, and carerelated documentation

• Ability to present information effectively to department heads, residents, families, physicians, and regulatory partners

 

Essential Functions

• Maintain compliance with all State and Federal Medicare/Medicaid rules, regulations, and published interpretations

• Review preadmission paperwork to ensure qualifying medical necessity

• Attend daily weekday standup meetings

• Coordinate Weekly Reimbursement Meetings with the Interdisciplinary Team to ensure reimbursement aligns with care delivery

• Audit clinical records to ensure documentation accuracy; educate and train staff on documentation guidelines

• Obtain and update Medicarequalifying diagnoses for Medicare Part A residents

• Initiate and update physician certifications for Medicare Part A residents

• Complete all MDS assessments within required timeframes for Medicare and Medicaid residents

• Review 24hour nursing reports to identify changes in resident condition

• Prepare for Medicaid audits and ensure documentation readiness

• Track resident benefit days, validate daily census, and coordinate information with the Financial Manager to ensure accurate billing

• Achieve budgeted reimbursement rate expectations

• Uphold Patient Care Management Systems and Financial Management Systems

• Ensure resident safety and compliance with Abuse Prevention policies

• Perform other duties as assigned
