Job Summary: At Lewis Park Post Acute, we are dedicated to providing exceptional care in both long-term and short-term post-acute settings. As a Registered Nurse (RN), you’ll lead and coordinate nursing care for residents recovering from illness or injury as well as those requiring long-term support. You’ll work as part of an interdisciplinary team, guiding care plans, supporting staff, and ensuring our residents receive the compassionate and professional care they deserve. Key Responsibilities: Provide and coordinate nursing care based on comprehensive assessments and individualized care plans. Administer medications and treatments safely and accurately. Monitor and assess residents’ conditions; respond to changes and report to physicians and family members as appropriate. Serve as a leader and clinical resource to Licensed Practical Nurses (LPNs) and Certified Nursing Assistants (CNAs). Oversee admissions, discharges, and transitions of care with a focus on continuity and quality. Maintain accurate documentation in compliance with regulatory standards and facility policies. Participate in care plan meetings, quality improvement initiatives, and ongoing training. Ensure infection control, safety, and emergency protocols are followed.
