Registered Nurse - Admissions Discharge - Full Time

Accountabilities Demonstrates the use of nursing process as a problem-solving model; utilizing their knowledge and experience to anticipate and plan to meet patient and family needs at time of admission, transfer, and discharge. Reviews all orders on new admission, transfers, and discharges. Communicates patients changes and condition with the assigned team members as indicated Communicates with patient/family within 24 to 72 hours of admission. Communicates daily with the interdisciplinary team after rounds. Collaborates with Primary RN, case managers/social services during the patient's stay, Assumes a proactive role in coordinating interdisciplinary discharge planning for a specific patient population, and facilitates outpatient resources that the patient may need in collaboration with the interdisciplinary team. Works with NP and other Providers throughout the stay to ensure patient will be ready for discharge by 12 noon. Demonstrates commitment to meeting the learning needs of patients and families. Utilizes appropriate resources to assist in meeting those needs and achieve positive patient outcomes Assists patient/family to maximize sense of control and actively participate in his/her recovery. Supports the patient discharge process in terms of medication reconciliation and finalizing discharge orders: completes the discharge summary, provides patient/family` education, and documents discharge teaching. Validates patient/family education and comprehension prior to discharge. Facilitates all follow-up appointments, and ensure ordered tests/procedures are scheduled prior to patients discharge. Communicates with patient/family 24 hours post discharge and documents encounter in UChart (telephone encounter) Conducts patient follow up at the following intervals (7, 14 and 30 days) post discharge and as needed. Documents encounter in UChart (telephone encounter) Is a liaison between medical/surgical team for patients/family post discharge. Demonstrates the knowledge and skills necessary to provide care appropriate to the age, and needs of the patients served on the assigned unit. Serves as a direct care provider, as appropriate, to ensure positive outcomes. Maintains a safe and clean environment that promotes safety of patients/family throughout all environments of care. Complies with regulatory standards including National Patient Safety Goals. Demonstrates creativity and innovation in approach to complex problem solving. Maintains professional appearance and adheres to proper dress code. Wears hospital I.D. badge and locator at all times, and promotes the standard of proper identification for peers and staff. Attends meetings as required Acts as role model and exhibits professional demeanor at all times. Maintains clinical competency. Demonstrates professional independence in the practice of nursing. Consults internal/external experts when appropriate. Maintains documentation of the continuing education requirements. CPR/BLS and ACLS are required. Maintains certification. Attends all mandatory in-services, and completes all computer based modules as assigned Actively participates in shared governance councils and professional organizations. Actively supports and participates in performance improvement activities. Seeks/accepts training to learn new skills and maintain current competencies. Maintains professional certifications/licensure in active status and in good standing. Practices within the scope and guidelines set forth by the Florida Board of Nursing. Attempts to resolves patient/physicians and visitors complaints and refers unresolved issues to managers. Demonstrates dependability/reliability by promptly reporting to work when scheduled. Float to other units in the role of ADT as necessary. Serves as mentor and role model. Compliance with UM, UHealth/UMHC and department policies and procedures Other duties as assigned. Supervision Received: Reports directly to ADT Supervisor Supervision Exercised: None Minimum Qualifications (Essential Requirements): Graduate from a nationally accredited school of nursing Bachelor’s degree required.
