APCM Registered Nurse Supervisor

The Role
We're hiring an APCM Supervisor to lead a remote team of up to 20 Registered Nurses delivering Advanced Primary Care Management to older adults with chronic conditions. You own the daily workflow, clinical quality, and operational performance of the team, balancing documentation and quality audits with performance coaching and productivity accountability. Reporting to the APCM/CCM Manager, you keep clinical standards and program metrics aligned as Cadence extends primary care into patients' homes between visits.
What You'll Do
- Lead the day-to-day workflow of a remote team of up to 20 Registered Nurses, holding the team accountable for the productivity and operational metrics that drive APCM program performance.
- Audit RN performance and clinical documentation to assess compliance, clinical quality, and adherence to APCM and chronic care management protocols.
- Coach RNs through structured feedback, translating audit findings and productivity data into targeted performance improvement.
- Own the performance of direct reports through regular 1:1s and performance improvement plans, resolving performance concerns as they surface.
- Partner with the Training Supervisor to close clinical education and ongoing training gaps across the team.
- Manage team scheduling, PTO, and time-off coverage to keep the remote team staffed against operational demand.
- Drive RN hiring by leading interviews and candidate selection for new team members.

What You Need
- Active, unencumbered compact Registered Nurse (RN) license, with additional single-state licenses preferred.
- Minimum of 5 years of practice as a Registered Nurse, ideally within Advanced Primary Care Management or chronic care management.
- Minimum of 1 year of supervisory or management experience with emphasis on clinical quality assurance, documentation auditing, and performance management.
- Demonstrated experience overseeing a high-volume, metric-driven operational environment, including interpreting productivity and operational data to manage workflow.
- Experience leading or supervising a remote clinical workforce.
- Fluency with AI-assisted clinical tools (such as Gemini, Open Evidence, or comparable platforms) and the ability to speak credibly with providers and patients about where AI supports care delivery and where human judgment remains non-negotiable.
Compensation
The anticipated salary range for this role is $80,000-$100,000 annually which is based on role scope, level, and location. In addition to base compensation, this role may be eligible for incentive compensation as part of the overall total rewards package. This position is remote and based in the United States. Actual compensation will be determined based on factors including experience, skills, internal equity, and applicable law.
