Patient Navigator - Diabetes Center - Days
Hourly Pay Range:
$21.08 - $31.62 - The hourly pay rate offered is determined by a candidate's expertise and years of experience, among other factors.Position Highlights:
Position: Patient Navigator
Location: Chicago, IL
Part time, 20 hours
Hours: Monday-Friday, 8am-4:30pm
Required Travel: no
What you will do:
Assist patients with access to social services and community-based resources; document all patient interactions
Assist patients with scheduling and appropriate referrals; this includes answering telephone calls, returning messages, scheduling appointments, communicating with other hospital departments to manage and obtain referrals.
Maintain inventory of office and medical supplies
Assist with grant functions as needed; this may include data entry and maintenance, and patient interaction
What you will need:
Education: High school diploma or equivalent
Experience: 1-yr clerical work experience
