Position Summary
Allegiance Benefit Plan Management is seeking a compassionate, proactive, and member-focused Onsite Nurse Care Coordinator to support the health and well-being of Kwik Trip employees and their covered dependents. This highly visible role serves as a trusted healthcare advocate, helping members navigate the healthcare system, access high-quality care, and engage in programs that improve health outcomes while reducing unnecessary healthcare costs.
As a key member of the Allegiance and Kwik Trip partnership, the Onsite Nurse Care Coordinator will collaborate with Kwik Trip leadership, Allegiance Care Management, client service partners, healthcare providers, and specialized clinical program vendors to identify opportunities for intervention and support. Through personalized outreach, education, care coordination, and advocacy, this role helps members make informed healthcare decisions and achieve better overall health outcomes.

Key Responsibilities
Care Coordination & Member Advocacy
Serve as an onsite clinical resource and healthcare advocate for Kwik Trip employees and covered dependents.
Support members with complex, chronic, high-risk, or emerging healthcare needs and coordinate care across providers, facilities, and programs.
Guide members through benefit questions, healthcare navigation, provider selection, and available treatment options.
Identify and address barriers to care, gaps in treatment, and opportunities to improve health outcomes.
Foster trusted relationships that encourage proactive healthcare engagement and informed decision-making.
Program Identification, Outreach & Engagement
Promote awareness and participation in targeted healthcare, wellness, and population health programs available to Kwik Trip members.
Identify members who may benefit from specialized programs using referrals, claims data, health risk indicators, care gaps, provider recommendations, and vendor insights.
Educate members on program benefits and available resources to encourage participation and sustained engagement.

Track outreach efforts, referrals, program participation, and member engagement outcomes.
Supported Programs
Mayo Clinic Complex Care Program- Supports eligible members and dependents with complex, rare, or difficult-to-diagnose medical conditions.
RightSite Program- Guides members to appropriate, high-quality, cost-effective sites of care, including specialty treatments and high-cost medications.
Wire Health- Provides behavioral health outreach, navigation, and support for members seeking mental health resources.
Abacus Health Solutions- Supports members with diabetes through education, engagement, and disease management resources.
Additional Clinical Programs- Supports Allegiance-sponsored wellness, care management, vendor, and population health programs as introduced.
Clinical Collaboration & Care Management
Partner with Allegiance Care Management nurses, care navigators, pharmacy teams, behavioral health resources, healthcare providers, and vendor partners.
Participate in case reviews and interdisciplinary discussions involving complex or high-risk member situations when possible.
Facilitate referrals to appropriate care management, disease management, behavioral health, wellness, and specialty programs.
Provide clinical insight and support to internal teams when appropriate.
Client Partnership Support
Build and maintain strong relationships with Kwik Trip leadership, Human Resources, wellness teams, and Allegiance stakeholders.
Participate in client meetings, strategic discussions, and healthcare program reviews.
Support onsite education initiatives, wellness campaigns, health promotion activities, and employee engagement efforts.
Identify trends, barriers, and opportunities that may improve member outcomes, program utilization, and overall plan performance.
Documentation, Privacy & Compliance
Maintain accurate, timely, and complete documentation of member interactions, referrals, care coordination activities, and follow-up actions.
Ensure strict adherence to HIPAA regulations and confidentiality requirements.
Follow all Allegiance clinical standards, care management protocols, and documentation guidelines.
Qualifications
Required
Current, unrestricted Registered Nurse (RN) license.
Minimum three years of nursing experience in care management, case management, population health, utilization management, clinical coordination, employee health, or a related healthcare setting.
Strong understanding of care coordination, chronic disease management, behavioral health, and healthcare delivery systems.
Exceptional communication, relationship-building, coaching, and member engagement skills.
Ability to work independently while collaborating effectively across multidisciplinary teams.
Strong organizational, time-management, documentation, and follow-through capabilities.
Preferred
Certified Case Manager (CCM), Board Certified Case Manager (CMGT-BC), or similar certification.
Experience working with health plans, self-funded employer groups, employee health programs, population health initiatives, or onsite health services.
Experience supporting complex case management, behavioral health coordination, specialty medication management, or chronic disease programs.
Familiarity with claims-based outreach, healthcare analytics, vendor management, and program engagement strategies.

As an extension of the Allegiance and Kwik Trip partnership, this position combines clinical expertise, relationship-building, and advocacy to make a meaningful difference in the lives of members while helping drive the success of employer-sponsored health initiatives.
If you will be working at home occasionally or permanently, the internet connection must be obtained through a cable broadband or fiber optic internet service provider with speeds of at least 10Mbps download/5Mbps upload.For this position, we anticipate offering an hourly rate of 38 - 64 USD / hourly, depending on relevant factors, including experience and geographic location.
This role is also anticipated to be eligible to participate in an annual bonus plan.
