Behavioral Health Specialist

Join our Collaborative Care team!

Trinity Health IHA Medical Group is seeking a compassionate and patient-focused Behavioral Health Specialist to join our Behavioral Health Collaborative Care team. In this role, you'll support patients experiencing behavioral health concerns through assessment, care coordination, brief evidence-based interventions, patient education, and ongoing symptom monitoring that improves outcomes and enhances the patient experience.

What You'll Do:

  • Support and closely coordinate behavioral health care with primary care providers, psychiatric consultants, and other treating providers

  • Screen and assess patients for common mental health and substance use disorders

  • Provide patient education regarding behavioral health conditions and available treatment options

  • Monitor patients for changes in clinical symptoms, treatment response, and medication side effects through in-person and telehealth interactions

  • Support psychotropic medication management by promoting treatment adherence and identifying barriers to care.

  • Deliver brief evidence-based behavioral health interventions such as Behavioral Activation and other structure therapeutic approaches.

  • Track patient outcomes and document clinical encounters.

What We're Looking For:

  • EDUCATION: Master’s degree or higher in Social Work, Counseling, or a related behavioral health field from an accredited university.

  • CREDENTIALS/LICENSURE: Valid Michigan clinical licensure (LMSW, LPC, LMFT, or similar applicable licensure).

  • MINIMUM EXPERIENCE: Experience assessing and treating common mental health and/or substance use disorders preferred. Experience utilizing evidence-based interventions such as CBT, PST, or Behavioral Activation preferred. Experience working with individuals with co-occurring behavioral and physical health conditions preferred.
