In this role, you will coordinate patient care, manage utilization reviews, and oversee discharge planning to ensure high-quality outcomes. You will act as the primary liaison between patients, families, healthcare providers, and insurance companies.Key Responsibilities
- Coordinate patient admissions, transfers, and discharge planning.
- Conduct utilization reviews to ensure appropriate level of care and insurance reimbursement.
- Collaborate with physicians, therapists, and nursing staff to develop individualized care plans.
- Communicate with insurance providers to obtain authorizations and manage concurrent reviews.
- Serve as an advocate for patients and families, guiding them through the healthcare continuum.
- Maintain accurate documentation in accordance with state, federal, and facility regulations.
- Active LVN license in the State of California.
- Minimum 1–2 years of case management or utilization review experience (preferred).
- Experience in skilled nursing (SNF) or long-term care settings highly desired.
- Strong knowledge of Medicare, Medi-Cal, and managed care insurance processes.
- Excellent communication, organization, and multitasking skills.
- Proficiency with electronic health record (EHR) systems.
