The Manager, Population Health will be responsible for managing a collaborative team-based strategy to manage complex patient services and initiatives, with a goal of achieving success in value based population health care outcomes.Job Description
Responsible for the day-to-day operations of a population heath team, which is flexible to the needs of the organization. Provide general oversight, management, and leadership for the team, work with executive leadership on strategies for population health services. Pursue and manage collaboration with other organizational departments, vendors, and partners within the Enterprise. Oversee data collection and evaluation of programmatic services. Promote population health initiatives within the team, the organization, and clients. Ability to assess business/organization needs, compare benefit/value and design/implement programs. Assists in the development, planning, and implementation of a system approach to improving and maintaining health standards and health outcomes in identified populations. Maintains communication and addresses the rapidly changing healthcare environment. Leads a team to be flexible and responsive to the organizational priorities and needs. Utilizes evidence-based practice in decision-making. Strategic and innovative thinker with proven ability to communicate and implement a vision and drive results. Demonstrates management, organizational, and interpersonal skills. Solves problems and executes on initiatives. Creates an environment where population health programs are valued and embraced. Leads and manages a team to respond to organizational needs and provides population health-based services. Evaluates population health performance measures; sets and communicates standards for improvement.Qualifications
Bachelor's degree in healthcare related field required; master's degree in public health or related healthcare area is preferred.Three years clinical and/or management experience in a healthcare and/or health plan setting is preferred. Experience and demonstrated success in case management, disease management, wellness, population health, public health, and/or community health programs.
