Job Description:
The in patient hospice nurse acts as an end of life specialist and highly skilled resource related to the general in patient level of care. The RN will take the lead to assess, plan, implement, coordinate, monitor and evaluate options to facilitate the continuum of care through discharge planning. The RN collaborates with the patient or patient’s family/representative, the physician, and members of the multidisciplinary team to facilitate progression-of-care; intervening appropriately to ensure that the plan of care and services provided are patient focused, high quality, evidence based, appropriate to patient needs, efficient, and cost effective. The nurse serves as a resource to the physicians and provides education and information on resource utilization, national and local coverage determinations, and use of hospital admission criteria. This role requires a high-level of leadership, interpersonal skills, and hospice knowledge.Education
- Required: Graduate of an accredited school of nursing
- Preferred: Bachelor's of Nursing
Experience
- Required: Minimum of 1 year hospital experience
- Preferred: Minimum of 1 year hospice experience
Skills
- Excellent interpersonal skills, including ability to be genuine and empathetic
- Excellent communication (verbal and written) skills with attention to detail
- Computer skills
- Ability to interact professionally and contribute to the interdisciplinary team
Licensure/Certification/Registration
- Current RN License in the state of Missouri
- Preferred: Certified Case Manager
