Telephonic Triage Specialist

We are seeking a Telephonic Triage Specialist to serve in a dual role (50% Team Lead / 50% Triage RN) supporting our after-hours hospice patients and teams across multiple states. This remote role is ideal for a registered nurse with strong clinical assessment skills, hospice experience, and a passion for mentoring others. As a key member of our virtual triage center, you will manage clinical calls, support field staff, and lead a team of remote nurses, ensuring excellent care coordination and quality patient outcomes.
Key Responsibilities – Team Lead (50%)
Supervise, coach, and provide real-time support to the triage team during assigned shifts
Monitor team performance, enforce quality assurance standards, and assist with training
Manage staff scheduling, assist with operational reporting, and support department workflows
Escalate critical care coordination needs and help ensure consistent coverage and timely care delivery
Participate in process improvement and SOP development to optimize triage operations
Serve as a point of contact for complex clinical issues, helping to resolve patient and branch concerns
Key Responsibilities – Triage RN (50%)
Respond to incoming calls from hospice patients, families, and caregivers
Use clinical assessment to determine level of care needed: phone support, nurse dispatch, or escalation
Provide guidance, reassurance, and education on symptom management and end-of-life care
Collaborate with interdisciplinary teams to maintain care continuity and support Plan of Care goals
Accurately document assessments and interventions in the EMR system
Provide remote triage coverage during evenings, weekends, holidays, and peak hours
About You
Education
Graduate of an accredited nursing program
Current RN licensure in a Nurse Licensure Compact (NLC) state
Bachelor of Science in Nursing (BSN) preferred
Experience
Minimum of 3 years of clinical experience
At least 2 years in hospice, oncology, or home health strongly preferred
Prior experience in telephonic triage required
Experience using EMRs and Microsoft Office Suite
Certifications
CPR certification
Hospice and Palliative Nurse Certification preferred
Valid driver’s license and insurance (in case of required branch visits)
Technical & Remote Work Requirements
Internet speed: Minimum 10 Mbps download / 5 Mbps upload
Quiet, dedicated home office
Ability to use a softphone, dual monitors, and headset for extended periods
Video capabilities for virtual meetings
Skills and Attributes
Advanced patient assessment and decision-making skills
Deep understanding of hospice care, including symptom management at end of life
Exceptional communication and empathy when speaking with families in crisis
Skilled in multitasking across multiple screens and systems
Strong leadership qualities with a proactive, solutions-oriented mindset
