The Medication History Technician (MHT) will perform medication histories through patient/family/significant other interviews for patients arriving via the Emergency Department to be admitted to the inpatient units. The role requires an appropriate level of understanding and training in medical terminology and medication related terms as well as regular communication with admitting providers and nursing staff in the Emergency Department.
What you will do
Perform medication histories on patients admitted to the hospital through the Emergency Department
Maintain regular communication with admitting providers, Emergency Department nursing staff and Emergency Department provider staff
Contribute to program optimization and provide feedback to Lead Medication History Technician and/or supervisor as needed, or as requested
Utilize the hospital’s electronic health record system to document the patient’s current medication list into the electronic medical record
Minimum Qualifications
Minimum High School Diploma or equivalent required
State of Connecticut Pharmacy Technician Registration required
Certification from the Pharmacy Technician Certification Board required within one year of date of hire
Knowledge of pharmaceutical terminology and metrics system
Minimum of two years of experience as a Pharmacy Technician in a retail setting or equivalent experience required
Bilingual in Spanish, preferred
Position Highlights and Benefits
Full time 40hrs M-F primarily days but some evening rotation, shift differential will apply
