Caregiver Navigator LOCATION: Remote

ABOUT IANACARE 
At ianacare, we’re on a mission to support family caregivers with innovative solutions. We partner with employers and health plans to provide comprehensive support that reduces caregiver stress, improves well-being, and enhances productivity. In 2025, we launched our partnership with healthcare providers on the CMS GUIDE Model to support caregivers of patients with dementia. Designed to integrate with existing care ecosystems, our GUIDE product offers a dedicated, adaptive experience that empowers caregivers by streamlining coordination and delivering essential support. We’re in an exciting growth phase and are looking for a passionate and organized person to join our team, drive successful partnerships within the healthcare sector, and manage new and existing employer clients.
ABOUT THE ROLE 
As a Care Navigator, you’ll be the expert guide for senior caregivers navigating non-clinical support and benefits (e.g., Medicare, VA, employer-sponsored programs, and community resources). You’ll build trust-based relationships, develop personalized action plans, and empower caregivers through phone, video, and chat interactions across six pillars: mental health, physical health, finance/legal, care management, work-life integration, and basic needs. 
WHAT YOU’LL DO 
Conduct 1:1 video and phone sessions (30 min) and chat to assess caregiver needs and goals
Develop and update individualized care plans focused on support and benefits navigation
Guide caregivers to entitlements, community services, and employer-provided programs (e.g., respite funding, adult day care)
Collaborate with interdisciplinary teams (e.g., social workers, case managers, primary
care providers) to coordinate non-clinical support
Document encounters, action plans, and resource referrals in our platform within 24 hours of each interaction
Monitor caregiver engagement; proactively reach out when participation drops or new needs emerge
Provide coaching via webinars, written resources, and videos to reinforce best practices in dementia caregiving Provides follow-up with clients and family to help ensure access to services and programs Maintains individual case records and complete documentation in addition to follow-up tasks
Become a Certified Dementia Practitioner within the first 90 days of employment
Conduct a variety of dementia education/support programs 
WHAT YOU’LL BRING
Demonstrates empathy and understanding of the challenges faced by caregivers and the loved ones they care for
You are a do-er, you live and breathe in to-do lists, and you are an overall proactive and organized individual with excellent project management skills
Possesses strong written and verbal communication skills, with the ability to clearly and effectively communicate with caregivers and interdisciplinary team members
Team player with a collaborative mindset and the ability to work effectively across different functions
Demonstrated resourcefulness and ability to troubleshoot or problem-solve independently
Comfortable using multiple tech platforms and navigating between systems
Comfortable working at a scrappy startup and managing multiple priorities
Detail-oriented and committed to delivering high-quality results
Proven expertise navigating senior benefits (Medicare, Medicaid, VA, LTC insurance, employer programs)
Commitment to HIPAA compliance and maintaining caregiver confidentiality
Strong documentation skills and comfort with tech tools (Zoom, Excel/Sheets, EHRs, chat platforms)
Ability to self-manage in a structured remote work environment, with a reliable, quiet workspace and consistent availability during shift hours
Ability to travel to headquarters biannually 
PREFERRED QUALIFICATIONS & EXPERIENCE
2+ years of professional experience in dementia care and geriatrics, or related non-clinical support roles (required)
Associate’s or Bachelor’s degree in Social Work, Nursing (LVN/LPN, RN), Case Management (CCM/CMC), or Community/Public Health Worker (MA, CHW, CHES)
Prior experience in telehealth, virtual coaching, or remote support roles
Familiarity with public and private benefits navigation (e.g., Medicare, Medicaid, VA, long-term care insurance)
Experience working with diverse populations, including those with limited English proficiency, low digital literacy, or socioeconomically disadvantaged backgrounds
Training or certification in dementia-specific communication techniques (e.g., Teepa Snow, Positive Approach to Care, Alzheimer’s Association programs) is a plus Bilingual in Spanish or Mandarin is a plus and strongly valued in serving diverse caregiver communities Location preference: Remote within the Eastern or Central Standard Time Zones 
CULTURE & COMPENSATION 
Hourly Rate: $21.00 – $26.00/hr—The final rate depends on your location and experience

SHIFT STRUCTURE & COVERAGE ianacare provides a 24/7 caregiver helpline, in addition to core navigator coverage from 8:00 a.m. to 8:00 p.m. ET, Monday–Friday. Navigator shifts are scheduled within this window as either three 12-hour or four 10-hour blocks a week to ensure reliable daytime support for  caregivers. Due to the nature of this role, availability during your assigned shifts is essential. Candidates must be able to commit to a consistent schedule within this time frame—flexible or ad hoc hours are not available for this position.

The Mission

We are looking for a high-octane Direct to Consumer Growth Marketing Manager to own the engine that connects Medicare-eligible caregivers with our clinical providers. You aren’t just “running ads”; you are architecting a high-trust journey. You will lead the strategy and execution of our paid acquisition channels (Meta & SEM) and optimize our HubSpot-driven nurture funnel to ensure every qualified lead results in a successful clinical enrollment call.

The Role: Strategy meets “In-the-Weeds” Execution

As our first dedicated growth hire, you will be a “Department of One” initially, responsible for:

  • Paid Media Mastery: End-to-end management of Meta Ads and Google SEM. You’ll handle the creative testing, audience layering (hyper-local targeting), and daily bid optimizations to drive high-intent caregiver lead forms.

  • Lead Generation & Landing Page Ownership: Designing, building, and A/B testing landing pages within HubSpot. You understand that a lead form is only as good as the page’s ability to filter for pre-qualified candidates.

  • The “Nurture-to-Call” Bridge: Designing the automated workflows (Email/SMS) that bridge the gap between “Form Submitted” and “Live Phone Call.” Your goal is to keep the lead “warm” and educated so they are ready to enroll the moment they speak to our clinician.

  • Data & Attribution: Maintain a source of truth. Ensure every lead from Paid and Influencer channels is correctly tagged, tracked, and attributed within HubSpot. Tracking the true ROI. You’ll move beyond “Cost per Lead” to focus on “Cost per Enrolled Member,” using HubSpot data to find which ad sets produce the highest-quality clinical conversations.

  • Marketing Operations: Manage (not create) weekly social assets, ManyChat flows, tracking links, and influencer collaborations. While assets are provided, you’ll handle the “last mile” of posting on our social channels.

The “Must-Haves”

  • Startup DNA: You have worked in a fast-paced startup. You don’t wait for a project manager to tell you what to do; you find the friction in the funnel and fix it.

  • The Tech Stack: Deep, hands-on experience with Meta Ads Manager, Google Ads, and HubSpot (Marketing & Sales Hub).

  • Strategic Empathy:You live and breathe consumer marketing. You can write copy that resonates with a stressed caregiver. You understand the nuances of the Medicare audience and how to build trust quickly in a local market.

  • Full-Stack Capability: You can write the ad copy, pick the image, build the HubSpot page, and set up the Zapier/Workflow logic yourself.

  • AI-Forward: You are excited about and actively use AI tools to create operational efficiencies and move faster. You have used Claude Opus and love testing new tools.

  • The Execution Mindset: You are comfortable wearing multiple hats—from technical attribution to light creative work in Canva. You are a doer.

It has come to our attention that there may be people posing as ianacare recruiters. These persons have been offering fraudulent employment opportunities to applicants and often asking for sensitive personal and financial information.

This type of fraud is normally carried out through online services such as false websites, or through fake emails claiming to be from the company. The fraudsters often request recipients to provide personal information and to make payments as part of their fake recruiting process.

Identifying Recruitment Fraud
