Job Summary and Responsibilities
As our Care Coordinator RN you will be a vital part of our Clinical Resource Management team, directly impacting patient care and outcomes. This role focuses on optimizing care coordination, ensuring efficient resource utilization, and fostering seamless patient transitions within our healthcare system. If you are a compassionate Registered Nurse with a passion for holistic patient management and a desire to make a tangible difference, join our dedicated team.
Every day you will engage directly with patients, providing comprehensive care coordination that touches their lives. Job Requirements
- Graduate of an accredited school of nursing and Minimum two (2) years of acute hospital clinical experience, upon hire or
- Masters Other Case Management or Nursing field. and 1 year of experience, upon hire and
- Registered Nurse: NE, upon hire and
- Basic Life Support - CPR
