Job Details

Position Summary

The Care Management Social Worker provides psychosocial assessment, discharge planning, and care coordination services for hospitalized patients. Working collaboratively with physicians, nurses, case managers, and community providers, this role helps ensure safe, timely transitions of care while connecting patients and families with appropriate resources and support services.

Responsibilities

  • Complete psychosocial assessments and identify barriers to discharge.
  • Develop and coordinate patient-centered discharge plans with the interdisciplinary care team.
  • Arrange referrals to home care, rehabilitation, skilled nursing facilities, hospice, behavioral health, and community resources.
  • Provide crisis intervention, counseling, and emotional support to patients and families.
  • Participate in interdisciplinary rounds and collaborate to facilitate timely, safe patient discharges.
  • Educate patients and families on available community resources, healthcare benefits, and advance care planning.
  • Document assessments and interventions accurately in the electronic medical record.
  • Advocate for patients while maintaining compliance with hospital policies, regulatory standards, and patient confidentiality.
  • Participate in quality improvement initiatives and perform other duties as assigned.

Qualifications

  • Master's Degree in Social Work (MSW) from an accredited program required.
  • New York State Licensed Master Social Worker (LMSW) required; LCSW preferred.
  • One year of hospital or healthcare social work experience preferred.
  • Strong communication, assessment, organizational, and care coordination skills.
  • Knowledge of discharge planning, community resources, and healthcare regulations.

 

Posted Salary Range

USD $41.32 - USD $56.76 /Hr.
