We are seeking a Manager of Centralized Intake to join our team. In this leadership role, you will report directly to the Senior Director of Care Coordination and oversee all referral vendor operations for subscribing market facilities. You will ensure timely responses to referrals, manage a team of Intake Coordinators, and drive process improvements that enhance efficiency and patient access to care.

 

 

Key Responsibilities

  • Monitor and manage all eReferral vendors for subscribing market facilities.

  • Lead Intake Coordinators, overseeing the acceptance and/or response to all eReferrals.

  • Maintain oversight of process execution, reporting, schedule management, and quality monitoring.

  • Maximize efficiencies in referral acceptance, non-admission processes, and pending patient management.

  • Provide training, coaching, and user support to Intake Coordinators.

  • Troubleshoot vendor or system issues and resolve slow response times.

  • Collaborate with regional sales, operations leadership, and agency personnel to align referral processes.

  • Manage and update agency profiles within the referral platform.

  • Ensure referrals are archived accurately after action.

  • Develop best practices for referral vendor usage across all locations.

  • Prepare and analyze reports to track portal activity and ensure data integrity.

  • Participate in after-hours on-call rotation and deliver training for new employees as needed.

About You

Qualifications

  • High school diploma or equivalent with customer service experience required; Bachelor’s degree in business, social work, psychology, nursing, or equivalent business experience preferred.

  • LPN/LVN certification preferred.

  • 3+ years of management experience in Centralized Admissions.

  • 5+ years of hospice and/or home health experience.

  • Strong organizational and customer service skills.

  • Hospice or medical office experience preferred.

  • Proficient in Microsoft Office applications (Word, PowerPoint, Excel, Outlook).

  • Ability to work in an interdisciplinary setting.

  • Detail-oriented with strong problem-solving abilities.

  • Skilled in managing business and clinical files systematically.
