This is a hybrid position, with the majority of work performed remotely. On occasion, the employee may be required to report to our Lakewood Ranch office for meetings, training, or other business needs.

This role consists of 8-hour shifts and supports the Tidewell and Empath Hospice service regions. The position is primarily focused on providing telephonic patient support and visits; there is no travel to patient homes, and patient interactions are conducted by phone rather than through video conferencing.

What you'll Do

  • Gathers information related to patient’s present condition/status, current and future plan of care, and identifies any barriers to optimal care.
  • Documents referrals to the Pathways team in the electronic medical record.
  • Provides resources, education and support to clients, family and care partners regarding decision making.
  • Advocates for and oversees the transition of care from Pathways to any services needed.
  • Communicates with internal colleagues including Empath Health physicians regarding client’s eligibility for services including hospice care.
  • Obtains and documents verbal certification of patient’s prognosis (eligibility).
  • Assesses the need for resources outside the Empath Health Integrated Network of Care.

What You'll Need

  • Education and/or Experience: Associate’s degree (A.A.) or equivalent from two-year college or technical school; or six months to one year related experience and/or training; or equivalent combination of education and experience.
  • Minimum two (2) years of hospice experience.
  • Licensure: Current valid Florida RN license.
  • Continuing education: As required for licensure.
  • Knowledge of hospice regulations such as admission/qualification criteria, reimbursement, and levels of care preferred.
  • Excellent, positive relationship building, outreach efforts, problem solving, organizational, time management, creativity, interpersonal skills, written and verbal communication a must.
