As a Care Access Specialist, you’re the patient’s introduction to KangarooHealth. You’ll enroll patients into both our Remote Patient Monitoring and Chronic Care Management programs, answer real questions, secure consent, and guide patients through onboarding. For CCM patients, you’ll also complete the first care call—ensuring they feel supported and connected from day one.This is not a scripted sales role. Patients need to trust you, not feel sold to. You earn that trust in the first call—by being honest, clear, and genuinely invested in helping them access care that improves their life.What You’ll DoEnrollmentMake outbound calls to eligible RPM and CCM patients on behalf of KangarooHealth’s partner provider groupsExplain our Remote Patient Monitoring and Chronic Care Management programs clearly and compassionately—what they are, why they matter, and what patients can expectAddress skepticism and concerns directly and truthfully—no scripts, no pressureSecure verbal consent and complete patient enrollment in KangarooHealth’s systemMeet daily enrollment targets while maintaining quality and accuracy on every callInitial CCM Care CallConduct the first CCM care call with newly enrolled patients, covering care plan review, chronic condition education, and patient goalsProvide health coaching and education on chronic condition self-management (HTN, DM, CHF, COPD)Identify and escalate clinical concerns or urgent patient needs to the nursing or provider teamAccurately document CCM time and call activity in the client’s EMR per billing protocolApply working knowledge of CCM billing codes (99490, 99439, 99487, 99489) to ensure compliant documentationWho We’re Looking ForCalm under pressure, clear under stress—you stay steady when a patient is frustrated or confusedYou listen before you speak and guide without pushing—patients trust you because you’ve earned itYou can explain a government program to a skeptical senior and gain their confidence in under five minutesOrganized and self-directed—you manage your call queue, hit goals, and don’t need to be micromanagedComfortable with volume—you can execute high-volume outreach daily without losing quality or warmthTech-fluent—you navigate CRMs, EMRs, spreadsheets, and digital workflows without frictionAccountability-minded—you follow the playbook, flag problems early, and ask smart questionsRequirementsActive LPN or LVN license with multi-state compact privileges3+ years of clinical experience in a hospital, family practice, or geriatric setting3+ years of EMR documentation experience; ability to learn new platforms quickly1+ year of remote work experience with patient phone engagementStrong understanding of chronic conditions and their management (HTN, DM, CHF, COPD)Familiarity with Medicare, CCM/RPM program requirements, and CMS billing codesTechnical fluency with G-Suite, Microsoft Office, Hubspot, Slack, Zoom, Epic, ECW, Athena, and similar platformsReliable high-speed internet, dedicated computer, and a private, HIPAA-compliant home workspaceAvailable to work 40 hours per week aligned with client clinic hours (9AM–6PM local time)Preferred ExperienceExperience with CCM, RPM, or other value-based care programsPrior call center or high-volume phone outreach experienceMedical Assistant certification (CMA, RMA, CCMA) or equivalent healthcare credentialBilingual fluency in...
