Director Care Management

POSITION SUMMARY:

The Director of Care Management is a Registered Nurse with strong leadership skills who assumes responsibility and accountability for the planning, organizing, development and coordination of the daily clinical activities of the Care Management staff and Social Work Discharge Planners. Provides administrative and regulatory details and support to the Care Management staff in an effort to accomplish the department’s goals and mission.

ESSENTIAL RESPONSIBILITIES / DUTIES: 

  • Provides direct care management care to patients and their families as needed. 

  • Manages patient flow, communicating to the Care Management staff to expedite admissions, discharges and transfers. 

  • Coordinates the Mentor Program for further development of Care Management staff. 

  • Intervenes in and provides consistent oversight for difficult cases, supporting the activities of the Care Management staff in LOS reduction, quality of care, legal, ethical issues. 

  • Works collaboratively with health care teams for implementation and revision. 

  • Serva as liaison between patients, families and the health care team. 

  • Directly supervise a team of Care Management and Utilization Review staff.  Hire, train, schedule, performance manage, discipline, support professional growth and development of staff 

  • Conducts extended-stay meetings with care management staff and care team stakeholders.       

  • Participates in continuing education through attendance and/or participation in in-services, grand rounds, seminars, conferences, staff meetings and yearly Joint Commission requirements.        

  • Develops and implements the fiscal plan in collaboration with finance team.  

  • Collaborates with MD and other disciplines in identifying and monitoring cost reduction efforts for patient care load and department.                   

  • Collaborates with all disciplines over non-wage reduction initiatives. 

  • Implements LOS goals and maximizes clinical efficiencies. 

  • Develops daily and monthly schedule.

JOB REQUIREMENTS 

REQUIRED EDUCATION AND EXPERIENCE: 

  • Master’s degree in Business or Nursing and 6 years of clinical experience, 2 years of which must be in leadership; or equivalent combination of education and experience.

PREFERRED EDUCATION AND EXPERIENCE (If none, please enter “N/A”): 

  • 4-5 years of leadership experience preferred. 

CERTIFICATIONS, LICENSES, REGISTRATIONS REQUIRED (If none, please enter “N/A”): 

  • By start date, active license to practice Nursing in the Commonwealth of Massachusetts.

KNOWLEDGE, SKILLS & ABILITIES (KSAs) (If none, please enter “N/A”): 

  • Strong leadership skills. 
  • Strong communication skills, in-depth knowledge of Care Management fiscal issues.   
  • Ability to make formal presentations relative to clinical, educational, and financial issues. 
  • Knowledge of QIO Medicare Process 

Compensation Range:

$118,500.00- $172,000.00

This range offers an estimate based on the minimum job qualifications. However, our approach to determining base pay is comprehensive, and a broad range of factors is considered when making an offer. This includes education, experience, skills, and certifications/licensures as they directly relate to position requirements; as well as business/organizational needs, internal equity, and market-competitiveness.
