Chronic Care Manager/Chronic Care Coordinator

Monday-Friday 8am-5pm

On-Site (Possibility of a hybrid work situation based on meeting monthly goals consistently)

$15-$17/hr

Requirements:

  • Minimum of 2 years of experience in an outpatient clinic setting as a patient caregiver, educator, or advocate

  • Certified Medical Assistant (CMA) preferred

  • Current Basic Life Support (BLS) certification

  • Strong interpersonal, communication, and organizational skills

  • Ability to work independently and adapt to a fast-paced environment

About the Role:
Phamily is assisting The NeuroMedical Center in hiring a Chronic Care Coordinator to work on-site using the Phamily Chronic Care Management platform. This role focuses on gathering and organizing patient data to identify unmet needs, engage patients in their care, and improve communication between patients and their care teams. Each Care Manager is expected to handle a 500-patient caseload with 300 billable patients by the end of the month.

This position is not eligible for visa sponsorship. 

Key Responsibilities:

  • Identify patients’ unmet needs and coordinate care management

  • Engage patients in their own care and support treatment interventions

  • Maintain ongoing communication with patients and their care teams

  • Record and organize medical information, including patient vital signs

  • Assist in treatments and administer medications as directed by physicians

  • Educate physicians and ancillary staff on care management protocols

  • Ensure compliance with HIPAA privacy regulations and patient confidentiality policies

  • React calmly and effectively in emergency situations

Physical Requirements:
This role requires standing and walking for extended periods, occasional lifting of up to 50 pounds, manual dexterity, and the ability to handle high-stress situations.
