Overview:
Healthy Families of the Blue Ridge is seeking a compassionate and motivated Home Visitor to join our Community Health team. This grant-funded role provides ongoing home visiting and prevention services focusing on child development, education, and resource coordination for at-risk families enrolled in the Healthy Families program. This position serves as a trusted liaison between Sentara Health and pregnant and postpartum individuals within the community.
The role integrates community health outreach with culturally responsive perinatal services to improve maternal and infant health outcomes, reduce disparities, and address social determinants of health. Responsibilities include providing education, advocacy, emotional support, care coordination, and system navigation throughout pregnancy, childbirth, and the postpartum period.
Education
High School Diploma or Equivalent (Required)
College coursework in child development, social work, or related field (preferred)
Certification/Licensure
Valid Driver's License with an acceptable driving record (Required)
Experience
1 year of experience working with children and families (preferred)
Knowledge of infant and child development (preferred)
Willingness to work with culturally diverse populations
Bilingual - English and Spanish (preferred)
Key Responsibilities:
- Provide regular home visits to participating families, focusing on building relationships, child development education, and parenting support
- Coordinate services and community resources for families
- Deliver prevention services tailored to meet the needs of at-risk families
- Maintain accurate and confidential records of family interactions and progress
- Collaborate with Healthy Families Supervisor and broader team to support program goals
- Engage in reflective practices and regular supervision
