The incumbent is responsible for psychosocial screening, assessment, diagnosis, treatment, and follow-up for transplant patients and living donors. In collaboration with physicians, transplant coordinators, TFC, PHS providers, and other community agencies, they assist in the management of psychosocial needs of the patients through all phases of transplantation and living donation. Participates in conflict resolution as necessary and acts as an effective problem solver.Conducts thorough and accurate transplant patient psychosocial evaluation that will establish an individual care plan to maximize transplant recovery while reducing the impact of psychosocial risk factors and engaging.Proficient at Psychosocial assessment documentation in patients Electronic Medical Record and ensures regulatory compliance.The Transplant Social Worker is responsible for the identification and assessment of psychosocial problems and will work with patients and families to achieve resolution or will provide referrals to the appropriate agencies for servicesThe Transplant Social Worker shall be available for hospitalized patients and their families throughout the episode of illness in the hospital.
