Job Summary and Responsibilities
As our Social Worker at CommonSpirit Medical Group - Community Care - Hixson, you will be a compassionate advocate, providing vital support and guidance to individuals and families facing complex challenges.Every day, you will conduct psychosocial assessments, develop care plans, provide counseling and crisis intervention, and connect clients to resources. You'll advocate for clients and collaborate with multidisciplinary teams to ensure holistic support and promote well-being.To be successful in this role, you will possess exceptional interpersonal skills, empathy, strong knowledge of community resources, and proven crisis management abilities, fostering positive change and client empowerment.
- Demonstrates competency in clinical knowledge and skills in assessing psychosocial consequences of thepopulation in a variety of practice settings, as well as implementing appropriate treatment plans.
- Develops a comprehensive bio-psychosocial assessment and intervention plan responsive to the patients andtheir families, staff needs and concerns.
- Develops an effective working relationship with patient/family to engage, collaborate, advocate and problemsolve in order to enhance their functioning.
- Utilizes, as a treatment modality, individual, couples and family and/or group psychotherapy.
- Demonstrates competency in accessing community and pharmaceutical resources to provide financial,transportation and supportive assistance.
- Directs patients and their families to financial/psychosocial resources available to them that are specific toGeriatric diseases and diagnosis, including resources for their children and family members.
