Description:

RN Care Manager

Full time - on site at 1440 Western Ave, Albany

This position is an ambulatory care registered nurse who will work focusing on our Trinity Health medical group patients.  The position will partner closely with providers, office staff, patients, families, and community resources to coordinate care across the healthcare continuum. Responsibilities include both in-person and telephonic care management, including:

Supporting patients with new diagnoses, complex medical conditions, and transitions of care.

Conducting comprehensive assessments and developing individualized care plans.

Providing patient education, coaching, and advocacy.

Collaborating in real time with providers to address urgent patient needs and remove barriers to care.

This is a full-time, on-site position in a busy, high-volume primary care practice. Our providers are passionate about ensuring patients receive timely access to care.

What you will need:

Associates Degree required, BSN preferred.

Previous experience required in care management at least one of the following settings: disease management, home health care, hospital or physician office setting.

Certification in Case Management (CCM) preferred.

Experience Requirements:

5 years of RN experience. 

Prior electronic medical record (EMR) experience required.  Previous project leadership experience required.  Possess strong community resource knowledge.  General knowledge of insurance plans and clinical resource management

Excellent oral and written communication skills.  Excellent customer service skills.  Ability to work independently, setting priorities to coordinate care plan efficiently.  Proven leadership skills and ability to work effectively in a team environment.  Highly organized and detail-oriented, ability to perform multiple tasks. Effective behavioral and educational strategies, including, but not limited to, motivational interviewing, teach-back method and self-management support. Recognize the role of cultural, social, economic and behavioral factors in accessibility, availability, acceptability, and delivery of public health services, and respond to diverse needs that are a result of these differences. Incorporate strategies for interacting with persons from diverse backgrounds (e.g. cultural, socioeconomic, educational, racial, gender, age, ethnic, and affiliation, mental and physical capabilities).

  

Pay Range: $74,880-$98,841

Pay is based on experience, skills, and education. Exempt positions under the Fair Labor Standards Act (FLSA) will be paid within the base salary equivalent of the stated hourly rates. The pay range may also vary within the stated range based on location.
