Job Description:

•  Manage and advance the clinical services of the pharmacy department to support a high level of patient care and the appropriate utilization of drug therapy

• Serve as a resource on infectious diseases for pharmacists, nurses, and physicians

• Provide direct patient care activities including disease-state management, pharmacotherapeutic and pharmacokinetic monitoring, adverse drug event (ADE) surveillance, and patient and staff education related to infectious diseases

• Document clinical interventions and patient care activities accurately and consistently

• Promote appropriate formulary use and coordinate the procurement of non-formulary drugs when clinically necessary

• Develop advanced clinical services, including pharmacy-managed protocols and medication use management programs — clinical guidelines, critical pathways, disease management, and drug use programs

• Conduct quality assurance initiatives, clinical pathway development, and assist with departmental readiness for regulatory audits

• Serve as a mentor and preceptor to staff pharmacists, pharmacy students, and pharmacy residents through example, training, and education

• Review pharmacy and organizational policies and procedures related to drug use

• Assist the Antimicrobial Stewardship Program leader in the implementation, maintenance, and advancement of stewardship initiatives to support organizational goals related to antimicrobial optimization, patient outcomes, and regulatory compliance

Pay Range:

$50.49 - $93.91
