How you’ll make an impact in this role
This is an exceptional opportunity for an experienced Family Medicine Nurse Practitioner or Physician Assistant to partner with Ascension Wisconsin and serve the local city, county, and school district workforce at the Racine Coalition EBC. In this unique employer-based clinic setting, you will combine primary care and acute management, building your own panel while establishing strong relationships with local client employers.
- Manage a Dedicated Primary Care Panel: Serve as the primary care provider for patients ages 2 and older, directing full-spectrum chronic disease management and proactive preventive care protocols.
- Deliver On-Demand Acute Care: Evaluate and treat acute, walk-in illnesses (such as sinus infections, colds, and minor injuries) with efficient diagnostic and clinical acumen.
- Drive Comprehensive Wellness: Perform preventative health assessments, screenings, and immunizations, utilizing on-site medication dispensing to ensure seamless, convenient patient care (prior dispensing experience is welcomed, but not required).
- Empower Through Health Education: Counsel and educate patients and families on treatment options, lifestyle modifications, and community health resources to foster long-term wellness.
- Lead Client & Community Relations: Utilize strong leadership skills to collaborate with client employer leadership, ensuring clinic services align with workforce health goals and community needs.
- Document Excellence: Capture every care journey with clinical precision, maintaining high-quality medical records that comply with network standards and specialty guidelines.

What minimum requirements you’ll need
Licensure / Certification / Registration:
- One or more of the following required:
- Nurse Practitioner credentialed from the Wisconsin Board of Nursing obtained prior to hire date or job transfer date.
- Registered Nurse credentialed from the Wisconsin Board of Nursing obtained prior to hire date or job transfer date.
- Preferred credential(s):
- BLS Provider. American Heart Association or American Red Cross accepted.
- Drug Enforcement Admin credentialed from the Drug Enforcement Administration.
Education:
- Master's degree of Nursing required.
What additional preferences we're seeking
- Experience: Ideally looking for 3–5 years of clinical experience in Employer-Based Solutions, Family Medicine, Urgent Care, or Occupational Health.
- Licensure & Certification: Certified Nurse Practitioner or Physician Assistant eligible for an unrestricted Wisconsin state medical license and DEA registration.
- Core Competencies: Strong leadership qualities, independent clinical confidence, and a passion for building lasting relationships with both patients and employer partners.
