Job Title: Per Diem Pharmacist
The Per Diem Pharmacist serves patients by preparing medications; giving pharmacological information to multidisciplinary health care team; monitoring patient drug therapies.
Roles & Responsibilities:
- Dispenses medications accurately and in a timely fashion.
- Resolves questions regarding formulary and non-formulary medications, following policies and procedures.
- Provides dosing and therapeutic recommendations that take patient age/kinetic based on literature.
- Performs clinical chart reviews on a scheduled basis.
- Performs Patient Care Unit inspections on a scheduled and timely basis.
- Prepares parenteral medications using sterile aseptic technique.
- Maintains adequate medication supply in the Pharmacy area and informs supervisor of impending shortages
- Intervenes and documents interventions to correct/ improve pharmacotherapy.
- Reports adverse drug events and medication errors occurring in area serviced.
- Takes responsibility for the operation of the assigned Pharmacy area and activities of all supportive personnel.
- Maintains pharmacological knowledge by reading professional publications, attending workshops, conferences and C.E. programs to maintain professional competency.
Requirements:
- Graduate from an accredited College of Pharmacy (ACPE).
- 1 year minimum as licensed Pharmacist
- Licensed Pharmacist-preferred

Compensation Statement
The Mount Sinai Health System (MSHS) provides salary ranges that comply with the New York City Law on Salary Transparency in Job Advertisements. The salary range for the role is $75.00 - $75.00 Hourly. Actual salaries depend on a variety of factors, including experience, education, and operational need. The salary range or contractual rate listed does not include bonuses/incentive, differential pay or other forms of compensation or benefits.
