As Clinical Resource Manager, you'll make a difference in residents' lives while guiding a dedicated clinical team.

Key Responsibilities:

  • Lead and coordinate resident assessments (MDS), ensuring compliance with federal and state regulations.

  • Champion quality care by developing individualized care plans and working closely with interdisciplinary teams.

  • Oversee Medicare and managed care referrals and ensure accurate documentation and care coordination.

  • Serve as a mentor, coach, and leader for clinical staff, promoting excellence in care delivery.

  • Act as Chairperson of the Interdisciplinary Care Plan Team and attend care conferences with residents and families.

  • Monitor clinical workflows, staffing levels, and resource utilization to meet resident needs efficiently and effectively.

What We’re Looking For:

  • Registered Nurse (RN) with a current, active license in this state.

  • AANAC certification (or willingness to obtain within 6 months).

  • Minimum 2 years of MDS experience preferred.

  • CPR certified and skilled in clinical documentation and assessment tools.

  • Strong leadership, communication, and collaboration skills.

Preferred Qualifications:

  • Bachelor of Science in Nursing (BSN).

  • Knowledge of Care Tracker, PPS Medicare criteria, and long-term care regulations.
