Promotes and maintains a safe, therapeutic milieu by providing appropriate supervision and feedback to the other direct care staff.  Ensures that rounds are assigned and precaution records completed in real time. Collaborates with the Resource and Referral Department to facilitate patient admissions and transfers.  Requests and provides thorough hand off communication upon admission, transfer or discharge and when sending or receiving boarding patients. Interacts with the patient, the patient’s family, and the appropriate health care providers to collect and analyze data regarding the patient’s physical, psychosocial, environmental, self-care, educational, and anticipated discharge planning needs.
