This division focuses on supporting individuals recovering from illness, surgery, or acute exacerbations of chronic conditions, allowing them to receive skilled care while maintaining independence in a familiar environment.Intermittent services are delivered on a part-time or as-needed basis and include skilled nursing, physical therapy, occupational therapy, speech-language pathology, and home health aide support. Care is tailored to each patient’s individualized plan, with an emphasis on restoring function, improving mobility, managing symptoms, and promoting self-care.The program primarily serves patients transitioning from hospital or post-acute settings, making it a critical component of the care continuum. Clinicians collaborate closely with physicians, case managers, and caregivers to ensure coordinated, seamless care. Frequent assessments and care plan adjustments are conducted to align with patient progress and evolving needs.Vanderbilt’s Intermittent Home Care team prioritizes quality outcomes, patient safety, and reduced hospital readmissions through evidence-based practices and proactive care management.
