Full-Time Registered Nurse (RN) Liaison - Community Hospice
The Community Hospice is seeking a compassionate and experienced Registered Nurse (RN) Liaison to join our Hospice Intake Team. This rewarding position serves as a key connection between patients, families, hospital staff, and hospice services, primarily within Albany Medical Center and other Capital Region hospitals.
Schedule: Monday through Friday, 8:00 AM - 4:30 PM

Position Summary:
As an RN Liaison, you will evaluate hospitalized patients for hospice eligibility and interest in hospice care, educate patients and families about the benefits and support available through hospice services, and facilitate a seamless transition to hospice care when appropriate.
Key Responsibilities
Assess patients for hospice eligibility and appropriateness of services
Provide education to patients, families, and healthcare professionals regarding hospice philosophy, services, and benefits.
Collaborate with hospital physicians and Hospice Medical Directors to confirm hospice eligibility.
Coordinate and communicate key clinical information to ensure a smooth transition to the hospice admitting nurse.
Work closely with hospital case managers, discharge planners, physicians, and other healthcare professionals.
Serve as a trusted resource and advocate for patients and families exploring end-of-life care options.
What you will need:
A current license to practice as a Registered Nurse in the State of New York
Associates Degree in Nursing, BSN preferred
1 year of recent acute care RN experience required
Hospice, Palliative Care or RN Liaison experience a plus but not required.
The RN must be able to communicate effectively, verbally and written
Valid Driver's License and Reliable vehicle
High level of interpersonal skills to establish and maintain relationships with patients, families, physicians, and coworkers
Understanding of Hospice principles preferred

Pay Range: $38.00-$50.18 Pay is based on experience, skills, and education. Exempt positions under the Fair Labor Standards Act (FLSA) will be paid within the base salary equivalent of the stated hourly rates. The pay range may also vary within the stated range based on location.
